i applied for Medicaid (Medicaid is called MedQuest in Hawaii, i choose HMSA as my provider because more doctors take HMSA than other providers although I'm still looking into UHC is United Health Care cause they have a lot of really good services for older people, people with disabilities and others who need transportation and are homeless)
So I called HMSA to change my behavior to Ohana Health Plan and I got Mental Health Case Manager and they set me up with Day treatment program at Care Hawaii.
Care Hawaii has a Day treatment program it's M W F 8 am-2 pm, T & Thurs 8 am - 12 pm (free lunch is a frozen pizza or hoagie pizza, frozen chicken sandwich, frozen fried rice bowl also I noticed it's from Sams Club frozen aisle.
You can pick which days you want to go when you go to the Care Hawaii intake you were referred to Care Hawaii Day Program by Ohana Health Plan's Mental Health Case manager.
Also your mental health case manager can set you up with group homes which is free housing, and all you have to do to stay there is see a psychiatrist on telehealth every three months and I was diagnosed with social anxiety or as a new patient tell your psychiatrist your there for meds management I take for social anxiety is paxil.
Also, along with a mental health case manager that can get you things on the AMHD community housing array PDF like group homes the Shelter Plus Care which is 30% of income and covers housing and mortgage
you can get Shelter Plus from the AMHD Community Housing Array PDF from a social worker or a mental health case manager three ways i guess:
1. type case manager by calling the I.H.S list and they meet you where you are homeless and fill out the vispidat, and on the vispidat is the s + c (Shelter Plus care) and also Group homes and transitional housing by KWO who can get you a Letter of Verification she might say that you are in the pooling for housing with the vispidat.
2. The vispidat can also be filled out by going to Partners in Care at 200 N vineyard they are on the second floor i just walked in they said usually you need an appointment but they will let me walk in , and there they filled out a vispidat for me and checked off the Group Homes there is also rapid rehousing box to check off and also s + c Shelter Plus Care.
3. The other way was when my mental health case manager referred me to Care Hawaii and I got a Housing Specialist from Care Hawaii, she filled out or updated the vispidat (that Partners in care already did for me) for me that they say they are connected to each other as tough updated it but, I don't think they are anymore... cause they are scared of each other.
